I have! Some people make a challenge run out of the second punishment, but nobody dares take on the third. o_o

not completely true. destroying a second altar gives the ball'n'chain which is necessary for said super hard run

however, destroying a third altar is used in high score runs. for reasons.

Well, that's the only way to make the ghost appear after you have the hedjet, but those are some very specific circumstances. Normally, it's a very, very bad idea.
